Pharmacy Glossary: ApoVWG and Pharmacy Terms Explained

Law and deadlines

Abschreibung auf MedizintechnikAbschreibung spreads the cost of a device across the years it is used instead of deducting it in the year of purchase. The useful life applied comes from official tables, so the number of years is not a matter of choice.

AkutversorgungAkutversorgung is the supply of a prescription only medicine in an acute situation under paragraph 48b AMG. The paragraph is in the statute, but the mechanism is not operable yet, so it is a term to prepare for rather than a service to offer.

AnschlussversorgungAnschlussversorgung is the continuation of a chronic medication by the pharmacy without a current prescription, under paragraph 48a AMG. It applies since 2 July 2026, covers the smallest pack only, and cannot be repeated without a new prescription.

ApothekenreformApothekenreform is the everyday term for the 2026 overhaul of German pharmacy law. In legal terms it means the ApoVWG, which has been in force since 2 July 2026 and widened both the services a pharmacy may offer and the rules it operates under.

ApoVWGApoVWG is the abbreviation for the Apothekenversorgung-Weiterentwicklungsgesetz, the German law reworking what pharmacies are allowed to do. It has been in force since 2 July 2026 and, among other things, permits trained pharmacists to carry out a venöse Blutentnahme.

AufbewahrungsfristenAufbewahrungsfristen are the periods a pharmacy must keep the records a blood draw produces. Consent, the request form, the result and the device logs fall into different categories, and the period follows what the document is rather than where it was written.

Aufklärung und EinwilligungAufklärung is the explanation given before a blood draw and Einwilligung is the consent that follows it. Both belong to the pharmacist carrying out the venöse Blutentnahme, and neither may be delegated to anyone else.

BehandlungsvertragDer Behandlungsvertrag is the agreement between the pharmacy and the person receiving a service. The pharmacy owes a service performed with proper care and documented properly, the person owes the agreed fee, and it is concluded when the service is accepted.

BeratungsraumBeratungsraum is the space a pharmacy uses for confidential advice. The Apothekenbetriebsordnung asks for consultation that cannot be overheard rather than for a closed room in every case, so whether a separate room is needed depends on the service offered.

BetriebshaftpflichtversicherungThe Betriebshaftpflichtversicherung covers damage a pharmacy causes to third parties while operating as a business. Before a blood draw service starts, the insurer has to confirm in writing whether that activity and a fainting customer are covered.

BiostoffverordnungThe Biostoffverordnung is the German ordinance governing work with biological agents. It requires a written Gefährdungsbeurteilung before the first blood draw, assigns the activity to a protection level, and obliges the employer to instruct staff regularly.

E-RechnungspflichtE-Rechnungspflicht is the obligation to issue invoices between businesses in a structured electronic format that software can read directly. A PDF sent by email does not satisfy it, because only a person can read the content.

FilialapothekeFilialapotheke is a branch pharmacy run by the same owner as the main pharmacy, with its own premises and a responsible pharmacist on site. For a service rollout each branch counts as a separate site rather than an extension.

FilialverbundA Filialverbund is a main pharmacy and its branches held by one owner. Procedures, training, purchasing and transport can be organised once for the whole group, while responsibility and staffing remain fixed at each individual site.

Gewerbesteuer bei apothekenfremden LeistungenGewerbesteuer is the municipal trade tax on commercial business activity. The question for a pharmacy is whether adding a service that is not the sale of medicines changes anything about how that activity is classified and assessed.

GoBD-konforme KassenführungGoBD-konforme Kassenführung means keeping books and cash records in electronic form so that every transaction is recorded completely, in the right order and without silent deletion, and so that the records can be produced in readable form during an audit.

Gutschein für GesundheitsleistungenA Gutschein is a voucher that is sold or given away and redeemed later. Whether a pharmacy may issue one for a health service is a separate question from whether it may issue one for a prescription medicine.

HaftungHaftung is legal responsibility for damage caused. Offering blood collection is a new activity for most pharmacies, so it is worth checking how it sits with existing insurance cover and what documentation would actually be available if a claim arose.

HeilmittelwerbegesetzThe Heilmittelwerbegesetz, abbreviated HWG, governs how health services may be advertised in Germany. It permits factual information about a pharmacy service and prohibits promises of cure and guarantees of success.

Hygiene bei der BlutentnahmeHygiene bei der Blutentnahme covers hand hygiene, skin disinfection, single-use materials, safe sharps disposal and the room itself. It is a condition of offering the service rather than an optional standard of care.

InvestitionsabzugsbetragThe Investitionsabzugsbetrag lets a business deduct part of a planned purchase before the purchase is made, moving the tax effect into an earlier year. It only holds if the investment is actually carried out within the permitted window.

IVDR und CE-KennzeichnungThe IVDR is the European regulation for in vitro diagnostics, and the CE mark shows that a test has passed the conformity route that regulation requires. It states what the test is approved for and who may use it, not how good it is.

MPBetreibVThe Medizinprodukte-Betreiberverordnung sets out the duties that attach to anyone who operates a medical device in Germany. A pharmacy running a measuring device is the Betreiber and carries those duties itself, not the manufacturer and not the supplier.

PreisangabenverordnungThe Preisangabenverordnung requires prices shown to consumers to be complete and unambiguous. For a pharmacy service that means one total price including tax, stated before the customer commits, and displayed where the service is offered.

Rx-Abgabe ohne RezeptRx-Abgabe ohne Rezept is the umbrella term for the narrow cases in which a pharmacy may hand out a prescription only medicine without a prescription in front of it. It covers two separate rules, not a general discretion.

SchiedsverfahrenSchiedsverfahren is the arbitration route that settles the terms and the fees for pharmaceutical services when the pharmacists' side and the payers' side cannot agree between themselves. It is why a service fee is a negotiated number rather than a fixed one.

StandardarbeitsanweisungA Standardarbeitsanweisung, often called an SOP, is a written instruction in a pharmacy's quality management system. Written Standardarbeitsanweisungen have to be in place before a pharmacy carries out its first venöse Blutentnahme.

TelepharmazieTelepharmazie means pharmaceutical services delivered remotely, for example a pharmacist supervising a branch by video. The word does not appear in the ApoVWG, and the law contains no general authorisation for it.

TRBA 250TRBA 250 is the technical rule for biological agents in healthcare and welfare. It translates the duties of the Biostoffverordnung into concrete practice, covering safety cannulas, disposal containers, protective equipment and the ban on recapping.

TSE-PflichtTSE-Pflicht is the requirement that an electronic till carries a certified technical security module which signs every transaction as it is recorded, so that entries cannot be changed or removed afterwards without the change being apparent.

Vertraulichkeit der KundendatenVertraulichkeit der Kundendaten is the pharmacy's duty of confidentiality over everything it learns and records while offering a health service. It covers who may see a result inside the pharmacy, how it is handed over and how it is stored.

VorsteueraufteilungVorsteueraufteilung is the division of input tax when a business has both turnover that carries Umsatzsteuer and turnover that does not. Anything bought for use on both sides has to be apportioned between them somehow.

ZweigapothekeA Zweigapotheke is a branch pharmacy operated to maintain supply in an area that would otherwise be underserved. It is the ApoVWG's answer to thin coverage, and it is not a pharmacy without a pharmacist.

Services and pay

AmortisationsdauerAmortisationsdauer is how long a new service needs to pay back what was spent to start it. In its simplest form it is the initial investment divided by the annual cash surplus that the service leaves once its own costs are covered.

ApothekenrechenzentrumEin Apothekenrechenzentrum is the settlement house that collects a pharmacy's claims against the statutory payers, checks them formally, submits them and pays the pharmacy out. It handles the reimbursed side of the business and nothing else.

Apothekenübliche DienstleistungAn apothekenübliche Dienstleistung is a health-related service a pharmacy may offer alongside medicines, typically paid for privately by the customer. It is separate from a pharmazeutische Dienstleistung, which the health insurers finance.

ApothekenwirtschaftsberichtThe Apothekenwirtschaftsbericht is the annual economic report the ABDA publishes on German pharmacies, covering turnover, costs, results and the number of pharmacies. Each edition replaces the previous one, so its figures need a yearly review.

BetriebsergebnisBetriebsergebnis is the operating result before tax, what remains of turnover after goods, personnel and all other operating costs. It is neither turnover nor the owner's income, since tax and private drawings still come out of it.

BetriebsvergleichA Betriebsvergleich is an anonymised comparison of one pharmacy's figures against peers of similar size and location. It shows where a business sits in a distribution, which is not the same as showing where it should be.

Betriebswirtschaftliche AuswertungBetriebswirtschaftliche Auswertung is the monthly report the Steuerberater produces from the posted bookkeeping. It sets turnover against costs by account and shows a provisional result for the month and the year to date, long before the annual accounts exist.

BotendienstA Botendienst is the delivery of medicines to a customer by the pharmacy that dispensed them. It is a supply service with its own rules, and it is not the route by which blood samples travel to a laboratory.

Break-even einer ZusatzleistungThe Break-even of a Zusatzleistung is the number of appointments per week at which the service stops costing money. It is the additional fixed cost of running it divided by the contribution each single appointment leaves behind.

BusinessplanBusinessplan is the written document a pharmacy prepares when it is founded, bought or expanded. It sets out the concept, the market, the people and the numbers, and it exists mainly so that a lender can judge the plan.

Direktabrechnung mit der PKVDirektabrechnung mit der PKV means the provider bills a private insurer directly instead of the customer paying up front. It requires an agreement with that insurer, which most pharmacy self pay services do not have.

EigenkapitalquoteEigenkapitalquote is equity expressed as a share of the balance sheet total. It shows how much of what the pharmacy uses belongs to it rather than to a bank or a supplier, and a lender reads it as a cushion against loss.

ErtragswertverfahrenErtragswertverfahren values a pharmacy on the earnings it is expected to produce rather than on the assets it owns. Past results are weighted towards the recent years, an owner's salary is deducted, and the remainder is capitalised.

Fixkosten und variable KostenFixkosten stay the same whatever the volume, from rent and core staffing to software and depreciation. Variable Kosten move with each unit sold or each appointment run, such as consumables, laboratory fees and additional working hours.

FixumThe Fixum is the fixed fee a pharmacy earns on each prescription pack, on top of a three percent surcharge. It rose from 8,35 Euro to 9,00 Euro on 1 July 2026 and rises to 9,50 Euro on 1 January 2027.

FördermittelFördermittel are subsidised loans and support programmes a pharmacy can use when founding, buying or investing. They usually reach the borrower through the house bank, and the application normally has to come before the investment starts.

GOÄGOÄ is short for Gebührenordnung für Ärzte, the fee schedule physicians use to bill privately paying patients. A pharmacy does not bill under the GOÄ, and it is often misunderstood as a price list for pharmacy services.

Ideeller WertIdeeller Wert is the part of a pharmacy's price that is neither equipment nor stock. It stands for the location, the customer relationships and the going concern, and it usually makes up the larger share of the price.

ImpfhonorarDas Impfhonorar is the fee a pharmacy receives for administering a vaccination. Pharmacy owners treat it as the yardstick for every other service, because it puts a known amount of revenue against a known number of staff minutes per appointment.

Kalkulatorischer UnternehmerlohnKalkulatorischer Unternehmerlohn is the notional salary an owner should count for their own work before calling the pharmacy profitable. It appears in no ledger of an owner run pharmacy, because the owner is paid out of whatever remains.

KapitaldienstfähigkeitKapitaldienstfähigkeit is the question whether a business produces enough surplus to pay the interest and the agreed repayment on its debt on schedule. A bank tests it before lending, because the loan is repaid out of that surplus.

KassenabschlagKassenabschlag is the statutory discount a pharmacy has to grant the statutory health insurers on each prescription package it dispenses. It is deducted at settlement, so it acts as a fixed drag on the prescription margin.

KaufpreisfaktorKaufpreisfaktor is the rule of thumb that expresses a pharmacy's price as a multiple of its earnings or of its turnover. It summarises a valuation that has already been done rather than producing one.

KundenfrequenzKundenfrequenz is the number of customers who come through a pharmacy's door in a defined period, usually read from the till as transactions per day or per opening hour. The trend matters more than the level.

Leasing oder Kauf von LaborgerätenLeasing and Kauf are the two routes by which a measuring device reaches the pharmacy. They differ in who owns the device, what is owed every month, what happens when the term ends and how the cost appears in the accounts.

LiquiditätsplanungLiquiditätsplanung sets out the cash expected in and out over the coming months on the dates it actually moves. It answers whether the account will cover the next payments, which is a different question from whether the pharmacy is profitable.

MedikationsanalyseA Medikationsanalyse is a structured review of everything a person takes, prescribed and bought over the counter, looking for interactions, duplications, dosing problems and adherence. It ends in a documented report, and the physician decides on any change.

MedikationsmanagementMedikationsmanagement is the continuing form of the medication review. The assessment repeats and the person is followed over time instead of being looked at once, which is what lets a second measured value be compared with the first.

PersonalkostenquoteThe Personalkostenquote is total personnel cost expressed as a share of Rohertrag rather than of turnover. It is the ratio that tells a pharmacy owner how much of the margin the team actually earns is spent on the team.

Pharmazeutische DienstleistungA pharmazeutische Dienstleistung, usually shortened to pDL, is a pharmacy service regulated in § 129 Sozialgesetzbuch V and paid for separately from the dispensing margin. The ApoVWG names ten of them directly in statute.

PreisuntergrenzeA Preisuntergrenze is the price below which a service should not be offered. The short run floor covers only the costs the appointment itself causes, while the long run floor also carries fixed costs and the owner's own time.

PrivatliquidationPrivatliquidation means invoicing the person who received a service directly, because that person is paying for it themselves. The pharmacy sets the price, issues an invoice in its own name and collects from the customer rather than from a Krankenkasse.

Rechnungsstellung an SelbstzahlerRechnungsstellung an Selbstzahler is the invoice a pharmacy issues to a customer who pays for a service themselves. It names both parties, describes the service, gives the date it was performed and the amount, and the customer may submit it onward.

RentabilitätsvorschauRentabilitätsvorschau is the forward looking profit forecast inside a business plan, usually covering three years. It projects turnover, cost of goods and operating costs and ends in an expected result for each year, before any money is spent.

RohertragsquoteThe Rohertragsquote is gross margin as a share of turnover. Owners tend to steer by the related figure of gross margin per customer instead, because it moves with decisions the pharmacy actually controls.

SonderbelegDer Sonderbeleg is the separate billing document a pharmacy uses to claim a pharmaceutical service. It names the pharmacy, the insured person, the service performed and the date, and it travels to the settlement house so the fee is paid from the pDL fund.

StundensatzkalkulationA Stundensatzkalkulation works out what one hour of pharmacy time really costs once social contributions and unproductive paid hours are included, and therefore what an appointment has to earn for the minutes it consumes.

SzenarioanalyseSzenarioanalyse means running the same plan through more than one set of assumptions, a cautious case, an expected case and a good case, so a reader can see how much of the result depends on the least certain number.

Umsatz je MitarbeiterUmsatz je Mitarbeiter divides a pharmacy's turnover by its staff, counted as full time equivalents. It measures output per head, and it only means anything when the headcount is defined the same way on both sides of the comparison.

UmsatzrenditeUmsatzrendite is the pre-tax operating result of a pharmacy expressed as a percentage of its turnover. It is the figure the trade press reports every spring, and it is a weaker steering instrument than it looks.

Diagnostics in the pharmacy

AbstrichAn Abstrich collects material from a body surface, most often the throat or the nose, with a swab. Whether the sample is usable is decided almost entirely by how it is taken and how quickly it reaches the laboratory.

Akkreditiertes LaborAn akkreditiertes Labor is a medical laboratory whose technical competence has been formally assessed against DIN EN ISO 15189. Accreditation is not the same as certification, which assesses a management system rather than analytical competence.

AntibiogrammAn Antibiogramm tests a pathogen that has already been identified against a set of antibiotics to show which of them still work against it. It answers the treatment question, not the question of what the infection is.

AntikoagulansEin Antikoagulans is the additive in a blood collection tube that prevents the sample from clotting after collection. EDTA, citrate and heparin each serve different analyses, while a serum tube contains none, because that sample is meant to clot.

AtemtestAn Atemtest measures a substance in exhaled air, usually after the person has swallowed a defined test substance. It needs no blood sample, and the preparation beforehand decides whether the result means anything.

Butterfly-KanüleEine Butterfly-Kanüle is a winged infusion set with flexible tubing, used for venous draws on small or difficult veins. It allows a shallow puncture angle and steadier handling, but it costs more and brings its own sample problems.

Diagnostische LückeThe diagnostische Lücke is the window between an exposure and the moment a test can detect it. A sample taken inside that window can come back negative even though the infection is already present.

DiskretionskarteDiskretionskarte is a small card or sign at the counter that a customer can point to in order to ask for a private word without saying anything out loud. It is a courtesy the pharmacy chooses, not a legal requirement.

EinmalstechhilfeEine Einmalstechhilfe is a single use lancing device for a capillary sample. It triggers once, retracts the blade automatically and locks, and the whole device then goes into the sharps container as one piece.

ErgebnisdokumentationErgebnisdokumentation is the form on which a pharmacy records what it measured or collected and what it handed over. It documents an act that was performed, with the person, the time, the staff member and the device, rather than an interpretation of the finding.

FIT-StuhltestA FIT-Stuhltest looks for blood in a stool sample that is not visible to the eye. The customer collects the sample at home, which makes the briefing at the handover the part that decides whether it is usable.

HämolyseHämolyse is the rupture of red blood cells in a sample, which releases their contents into the serum or plasma. It makes several analyses unreportable and it is usually caused during collection rather than at the laboratory.

HygieneplanA Hygieneplan is the written plan stating which surfaces, hands and instruments are cleaned or disinfected at a draw or measuring station, with which product, how often and by whom. The Betriebsanweisung for the activity belongs beside it.

Ikterische ProbeAn ikterische Probe is a sample whose serum or plasma is strongly yellow to brown because it carries a lot of bilirubin. The laboratory notes the discolouration because it can interfere with certain measurements.

ImmunoblotAn Immunoblot is a confirmatory test that shows which individual antibodies are present in a sample. It is used after a screening test has reacted, to establish whether that first result holds.

Interne QualitätskontrolleInterne Qualitätskontrolle is the routine measurement of control samples with known target values on the pharmacy's own device, run on the pharmacy's own schedule, to confirm that the device and the current reagent lot are still measuring correctly.

KalibrierungKalibrierung sets a measuring device against material of known value so that its readings are traceable to a reference. It fixes the scale on which results are read, while a control measurement only checks that the scale still holds.

KanülenabwurfbehälterA Kanülenabwurfbehälter is the puncture proof and break proof container into which used cannulas and lancets are dropped immediately after use. It stands at the draw station, is closed at the fill line and goes to disposal sealed.

Kapillare BlutentnahmeA kapillare Blutentnahme takes a small blood sample from the fingertip or through a device worn briefly on the upper arm. It needs no venous puncture and it suits a narrower range of analyses than a venous sample.

Lipämische ProbeA lipämische Probe is a blood sample whose serum or plasma looks cloudy or milky because it carries a high load of fat particles. The laboratory flags it, and for several analyses the turbidity makes a result unreportable.

Nachforderung aus der RestprobeNachforderung aus der Restprobe means asking the laboratory to run a further analysis on sample material it already holds. Whether it is possible depends on stability, remaining volume and tube type, and it needs fresh consent from the customer first.

NüchternheitNüchternheit means the person has not eaten for a defined period before a blood draw, usually 8 to 12 hours, with water permitted. It matters for some analyses such as Nüchternglukose and not for others.

PCRPCR stands for polymerase chain reaction. It copies a specific piece of genetic material many times over until there is enough to detect, which is why it can find a pathogen while the amount present is still very small.

Point-of-Care-TestA Point-of-Care-Test is a rapid test carried out where the person is, with the result available immediately. It suits screening and orientation, and it does not reach the analytical range or precision of an accredited laboratory analysis.

PräanalytikPräanalytik covers everything that happens to a blood sample before it is analysed, from preparation and identification through collection, storage and transport. Most laboratory errors arise here rather than in the analysis itself.

ProbenkennzeichnungProbenkennzeichnung is the labelling of the tube so that the sample can be matched to the right person and the right request. It happens at the person immediately after collection, and an unlabelled tube is discarded rather than guessed at.

ProbenlogistikProbenlogistik covers the handling of a blood sample between collection and the laboratory bench: storage, packaging, transport and timing. Analyses have stability windows, and a sample that misses its window is no longer reportable.

ProbenstabilitätProbenstabilität describes how long a sample stays analysable after collection. The window differs for every parameter and is shortened by time, temperature, light and handling, so the binding figures come from the laboratory specification.

Punktionsstelle an der FingerbeereThe Punktionsstelle is the spot chosen for a capillary puncture, at the side of the finger pad rather than its centre or tip. Site choice, a warm hand and blood that flows without pressure decide whether the sample is usable.

QualitätssicherungQualitätssicherung is the set of checks that make a result trustworthy: trained people, maintained devices, written procedures and external comparison. In a pharmacy it splits between what the laboratory guarantees and what the pharmacy itself controls.

Reaktives TestergebnisA reaktives Testergebnis means a screening test has reacted. It is a signal to confirm, not a diagnosis, and in a pharmacy it is handed on rather than explained at the counter.

ReferenzbereichA Referenzbereich is the range of values a laboratory reports as usual for a healthy comparison group. It belongs to the analysing laboratory, it differs between laboratories and methods, and a value outside it is not a diagnosis.

Reihenfolge der BlutentnahmeReihenfolge der Blutentnahme is the fixed order in which the individual tubes are filled during a venous draw. It exists because additive can carry over from one tube into the next, and the binding order is the one the receiving laboratory publishes.

SchnelltestEin Schnelltest is a test that gives a result on the spot, within minutes, without the sample being sent away. It is the plain word for what the technical literature calls a Point of Care Test, and it answers one narrow question.

Selbsttest oder LabortestSelbsttest oder Labortest compares a test bought off the shelf with a sample drawn in the pharmacy and analysed in an accredited laboratory. Each answers a different question, one narrow and immediate, the other broad and standardised.

Serum-Gel-RöhrchenEin Serum-Gel-Röhrchen is a blood collection tube holding a clot activator and a separating gel. After the sample has clotted and been centrifuged, the gel settles between the cells and the serum and keeps the two apart.

SerumgewinnungSerumgewinnung is the sequence that turns a drawn sample into serum, meaning complete clotting, centrifugation and separation from the cells. Where a pharmacy does not centrifuge, the laboratory carries it out and the pharmacy protects the sample until then.

SystemkontrolleSystemkontrolle is the routine check that the whole measuring system still works, meaning the device, the strip or cassette, the reagent and the person operating them, rather than any single part of that chain on its own.

TrockenblutTrockenblut, in English dried blood spot, is capillary blood dried onto a filter card. It travels by ordinary post without a cold chain, and it supports a narrower set of analyses than a liquid sample.

Venöse BlutentnahmeA venöse Blutentnahme is the drawing of blood from a vein. Since 2 July 2026 the ApoVWG permits it in pharmacies, carried out by a licensed pharmacist who has completed an ärztliche Schulung, on people aged 18 or over, for diagnostic purposes only.

Roles and training

Anamnese vor der BlutentnahmeAnamnese vor der Blutentnahme is the short structured history a pharmacy takes before a draw, covering anticoagulation, clotting problems, previous fainting and the arm to avoid. It establishes whether to draw at all and is a screening step, not a diagnosis.

ArzneimitteltherapiesicherheitArzneimitteltherapiesicherheit, usually shortened to AMTS, is the aim of making medication as safe as it can be along the whole path from prescribing to taking. The pharmacy holds one specific part of that path.

Ärztliche SchulungAn ärztliche Schulung is the physician-led training a pharmacist has to complete before carrying out a venöse Blutentnahme in a pharmacy. Successful completion has to be confirmed, and without it the draw is not permitted.

CubitalveneThe Cubitalvene is the vein in the crook of the elbow. It is the standard site for a venous blood sample because it sits close to the surface, is easy to find and is well away from arteries and nerves.

ImpfstatusThe Impfstatus is the record of which vaccinations a person has had and when. Pharmacies meet the question daily, both because they may vaccinate and because the record is so often incomplete.

MustercurriculumThe Mustercurriculum is the model curriculum for the ärztliche Schulung that prepares pharmacists to carry out a venöse Blutentnahme. It is due by 2 November 2026, and it is not a precondition for offering the service.

NadelstichverletzungA Nadelstichverletzung is a puncture injury in which a used cannula or another sharp object breaks the skin of a member of staff. In a pharmacy that draws blood it counts as an Arbeitsunfall and follows a fixed reporting route.
